What is Migraine?

Migraine is a neurological condition characterised by intense, throbbing headaches that typically affect one side of the head. It is far more than just a headache — migraine attacks can last from 4 to 72 hours and are often accompanied by nausea, vomiting, and extreme sensitivity to light and sound.

Migraine affects approximately 10–15% of the global population, is three times more common in women than men, and is one of the leading causes of disability worldwide. In India, millions of working adults suffer from recurring migraine episodes that significantly impact their productivity and daily life.

Types of Migraine

  • Migraine without Aura (Common Migraine): The most common type. Severe, throbbing, one-sided headache lasting 4–72 hours, with nausea and light/sound sensitivity. No preceding neurological symptoms.
  • Migraine with Aura (Classic Migraine): Preceded by a set of reversible neurological symptoms (aura) such as visual disturbances (flashes, zigzag lines), tingling in the face or hands, or temporary speech difficulty. Aura usually lasts 20–60 minutes before the headache begins.
  • Hemiplegic Migraine: A rare type where temporary weakness or paralysis occurs on one side of the body during or before the headache phase.
  • Chronic Migraine: Headaches occurring on 15 or more days per month for more than three months, with at least 8 days having migraine characteristics.

Symptoms

A full migraine episode typically passes through four stages, though not everyone experiences all of them:

  1. Prodrome (1–2 days before): Mood changes, food cravings, neck stiffness, frequent yawning, increased thirst.
  2. Aura (if present): Visual phenomena, numbness, tingling, weakness, or speech difficulty.
  3. Attack: Throbbing or pulsating pain usually on one side of the head; nausea or vomiting; sensitivity to light (photophobia), sound (phonophobia), and sometimes smell (osmophobia); pain worsening with physical activity.
  4. Postdrome: A period of fatigue, confusion, and mild head pain for up to 24 hours after the headache resolves.

Triggers

Identifying and avoiding personal triggers is a key part of migraine management. Common triggers include:

  • Hormonal changes: Oestrogen fluctuations during menstruation, pregnancy, or menopause in women
  • Food and drink: Aged cheese, alcohol (especially red wine), chocolate, caffeine (or caffeine withdrawal), processed foods with MSG or nitrates
  • Stress: Physical or emotional stress and its aftermath (the “let-down” effect)
  • Sleep changes: Too much or too little sleep; jet lag
  • Environmental factors: Bright or flashing lights, strong smells, loud sounds, changes in weather or barometric pressure
  • Medications: Oral contraceptives, vasodilators (e.g., nitroglycerin)
  • Dehydration and skipped meals
  • Neck and postural strain
